Combine sugar, honey, cocoa, heavy cream, and milk in a small saucepan and bring to a simmer over medium heat. Allow to simmer 2-3 minutes until slightly thickened, stirring frequently.
Remove sauce from heat and immediately whisk in chocolate, vanilla, and salt until chocolate is smooth and melted.
